Deliver compassionate, remote care as a Family Nurse Practitioner with a mission-driven organization transforming how patients access healthcare. You’ll conduct virtual wellness visits for individuals in their homes, working closely with in-home clinical team members to assess health status, review medications, and support long-term well-being.
What You’ll Do
- Begin each day by reviewing your schedule and ensuring technology is ready for seamless virtual visits
- Build trusting relationships with patients from diverse backgrounds through empathetic communication
- Perform comprehensive health risk evaluations and cognitive screenings
- Collect patient histories, interpret findings, and support clinical decision-making
- Collaborate with field-based clinicians to assess vital signs and coordinate necessary services
- Arrange lab work, including blood draws, and manage follow-up needs
- Provide education on health maintenance and preventive care strategies
- Coordinate referrals to specialists and community support programs
- Ensure accurate, timely documentation in compliance with clinical and privacy standards
What We’re Looking For
- Current certification and active licensure as a Family Nurse Practitioner in Michigan
- Strong commitment to patient-centered care, confidentiality, and ethical practice
- Proficiency with digital tools, including mobile devices and secure messaging platforms compliant with HIPAA
- Excellent communication skills, both written and verbal
- Ability to work independently while collaborating within a multidisciplinary team
