We are seeking a licensed Family Nurse Practitioner to deliver high-quality virtual care and lead wellness initiatives for patients in their homes. What You’ll Do
- Begin each day by reviewing patient cases and ensuring digital tools are ready for use
- Use secure, modern platforms to manage your daily schedule and patient interactions
- Build trusting relationships with individuals from diverse backgrounds during virtual visits
- Collect health histories, conduct risk assessments, and review medications
- Perform cognitive evaluations and deliver personalized health education
- Collaborate with in-home clinical staff to assess vital signs and coordinate services
- Arrange lab work such as blood draws through coordinated field support
- Address individual patient needs through tailored interventions
- Manage preventive care, medication plans, and specialist referrals
- Connect patients with local resources and support networks
- Ensure accurate and timely documentation of all clinical findings
What We Require
- Current certification and active license as a Family Nurse Practitioner in KY, MI, OH, or IL
- Demonstrated commitment to professionalism, empathy, confidentiality, and ethical practice
- Ability to think critically and adapt quickly in a fast-evolving environment
- Experience with digital charting systems and HIPAA-compliant communication tools
- Strong written and verbal communication skills
- Capacity to work independently while collaborating effectively within a clinical team
- Successful completion of a national background check and license verification
