What You'll Do
As a Family Nurse Practitioner, you will lead virtual patient visits from a remote setting, guiding care through real-time collaboration with in-home clinical staff. You’ll begin each day by reviewing your patient schedule and ensuring your technology is ready for seamless communication. Using secure digital tools, you’ll assess patient health, review medications, and conduct cognitive screenings during live interactions.
You’ll build trust with patients from diverse backgrounds, gathering key health information and guiding preventive care strategies. Working closely with field-based clinicians, you’ll interpret vital signs, recommend lab work such as blood draws, and make informed decisions about patient needs. Your role includes educating patients, managing chronic conditions, adjusting treatment plans, and connecting individuals with appropriate specialists or community services.
Accurate and timely documentation is essential, ensuring continuity of care and compliance with clinical standards. You’ll also contribute to improving patient outcomes by identifying risks early and delivering coordinated, patient-centered interventions.
Requirements
- Board certification as a Family Nurse Practitioner in GA, NC, SC, FL, or MS
- Active Family Nurse Practitioner license in California
- Demonstrated professionalism, integrity, and commitment to patient confidentiality
- Proven ability to work independently while collaborating effectively with clinical teams
- Comfort using laptops and mobile devices for electronic charting
- Experience with HIPAA-compliant messaging platforms for care coordination
- Strong communication skills, both written and verbal
- Adaptability in fast-moving, evolving environments requiring creative problem-solving
- Successful completion of a national background check and license verification
