What You'll Do
Act as the primary contact for patients entering the care program, guiding them through onboarding and maintaining consistent communication throughout their journey. You’ll coordinate Comprehensive Health Evaluations (CHE) by scheduling in-person and telehealth appointments, sending reminders, and managing reschedules based on patient needs and clinic availability.
Reach out to patients, caregivers, and clinical partners via phone to confirm appointments, gather feedback, and address concerns. When necessary, escalate issues to nurses or providers to ensure timely resolution. Build trust through empathetic communication, helping patients overcome barriers to participation in care initiatives.
Work closely with Nurse Practitioners and Integrated Care Nurses to support patient progression through care pathways. Assist with non-clinical screenings, maintain accurate records, and ensure all documentation reflects current patient status and interactions. Prioritize outreach based on urgency to maximize impact and improve engagement rates.
Connect patients with available resources and programs they qualify for, supporting enrollment in services that enhance care access. Communicate regularly with internal teams and external partners to coordinate care activities and ensure seamless transitions across care stages.
Requirements
High School Diploma or GED is required. Prior experience within Integrated Kidney Care (IKC) is mandatory. You must have a dedicated home workspace that is quiet and private, equipped with reliable high-speed internet—Ethernet connectivity may be required for system stability.
Proficiency with Microsoft Office Suite, including Word, Excel, PowerPoint, and Outlook, is essential. Strong verbal and written communication skills are critical, along with active listening and the ability to engage patients motivationally. You should be highly organized, capable of managing time effectively, and comfortable working independently while collaborating with a care team.

Work Environment
This is a full-time remote position with a flexible daily start time between 7:00 and 8:00 AM. There are no night shifts, weekends, or on-call requirements. The role may transition to a hybrid-remote model in the future. You must be located in or near Bellingham, WA, and able to meet all technical and workspace requirements for remote work.
