About the Role

VON Canada is looking for a Directing Registered Nurse (DRN) for its eHomecare service, working nights and weekends on a part-time (0.6) basis. In this remote role, you will provide clinical leadership and manage care for pediatric, palliative, and acute medically complex clients with conditions like COPD and CHF, directing Personal Support Workers in real-time using a collaborative care model.

What You'll Do

  • Provide remote assessment and management of care services to clients in their homes.
  • Provide clinical direction and assign tasks within the unregulated care provider's practice statement.
  • Maintain accurate client clinical records in both digital and paper formats.
  • Maintain patient assignment within the virtual ward.
  • Work collaboratively with all team members to ensure all patient care needs are met.
  • Utilize effective communication in daily huddles and in response to emails and phone calls.
  • Communicate with outside service provider agencies and Case Managers as appropriate.
  • Participate as a valued member of the multi-disciplinary health care team.
  • Provide clinical leadership, direction, and mentorship within the health care team.
  • Participate in and facilitate a collaborative team-based environment.
  • Establish and maintain relationships with local health care providers.
  • Participate in CNO Reflective Practice.
  • Uphold VON standards and eHomecare-specific processes.
  • Participate in internal projects and ongoing improvement initiatives.
  • Demonstrate professionalism in a diverse working environment.
  • Demonstrate adaptability in managing change in a fast-paced virtual environment.

What We're Looking For

  • Registration and good standing with the College of Nurses of Ontario.
  • A CNA Certificate in Palliative Care OR completion of both the 'Fundamentals of Palliative Care' and the 'Comprehensive Advanced Palliative Care Education (CAPCE)'.
  • For Pediatric DRNs: extensive hands-on experience in pediatric care provision.
  • Recent palliative, pediatric, and/or acute medical experience with COPD and CHF patients.
  • Demonstrated advanced assessment skills.
  • Experience in gerontology and/or medical-surgical nursing.
  • Competencies in observation, independent assessment, health education and promotion, and familiarity with COPD/CHF clinical pathways.
  • Digital and electronic health experience, including EMR and mobile technology.
  • Proficient nursing skills with the ability to read and interpret electronic technical information related to client outcomes.
  • Critical thinking, problem-solving, and multi-tasking skills.
  • Ability to work in a multidisciplinary virtual team setting and independently.
  • High-speed internet access, computer skills, and a private home office area.
  • Eligibility to successfully obtain a Government of Canada security screening certificate.
  • A valid CPR C / First Aid certification.
  • Access to a personal vehicle for business use, a current driver’s license, and proof of vehicle insurance.
  • A current and original copy of a satisfactory Criminal Records Check upon employment start.
  • Ability to wear Personal Protective Equipment (PPE).
  • A Vulnerable Sector Search and/or Child Abuse Registry Check may be required.

Nice to Have

  • Experience working in a community-based health care system.
  • Ability to speak French (an asset in French Designated areas).
