Responsibilities
- Support patient registration and enrollment through community outreach and proactive phone contact.
- Reach out to patients before appointments via virtual or in-person methods and conduct initial intake assessments.
- Work with a designated group of patients to guide care coordination, manage appointment details, assist with prescriptions, lab coordination, referrals, care plan follow-up, and distribute educational resources.
- Identify social determinants of health needs and connect patients to relevant support programs, including care planning, community referrals, coaching, and transition planning.
- Partner with interdisciplinary care teams to contribute to patient care strategies, prioritize requests, and address complex health challenges.
- Record all interactions with patients and care team members in multiple digital systems and platforms.
- Engage in community activities to build trust, strengthen patient involvement, and develop relationships with community partners, including leading group education and outreach.
- Deliver health education and information in a culturally and linguistically appropriate manner.
- Help patients enroll in public assistance programs, schedule appointments, manage referrals, and maintain consistent care access.
- Guide patients in setting personal health goals using motivational interviewing techniques.
- Carry out individual assessments to identify social needs affecting health outcomes.
- Offer emotional and practical support by actively listening to patient concerns and connecting them to appropriate services.
- Represent the organization at community events to promote engagement and visibility.
- Coordinate internal clinical services to ensure seamless patient care delivery.
