About the Role
This role focuses on connecting with community stakeholders to identify health needs, coordinate outreach events, and strengthen partnerships that support patient engagement and long-term wellness initiatives.
Responsibilities
- Establish and maintain relationships with local community organizations and leaders
- Plan and execute health outreach events and educational workshops
- Identify community health trends and gaps in service delivery
- Collaborate with internal teams to align outreach with patient needs
- Track and report on community engagement metrics and outcomes
- Support the development of culturally appropriate health materials
- Facilitate referrals to medical and social services
- Represent the organization at community forums and public meetings
- Coordinate transportation and access solutions for patients
- Assist in evaluating program effectiveness and community impact
- Promote preventive care and wellness programs locally
- Engage faith-based and nonprofit groups in health initiatives
- Gather community feedback to inform service improvements
- Support recruitment efforts for health programs
- Maintain accurate records of community interactions and events
- Respond to community inquiries and direct to appropriate resources
- Assist in grant-related community reporting when needed
- Advocate for patient needs within the healthcare system
- Help onboard new community partners
- Support multilingual communication efforts as needed
- Participate in staff training on community engagement strategies
- Ensure compliance with privacy and data protection standards
- Work cross-functionally to address social determinants of health
- Contribute to quarterly community impact summaries
- Stay informed about local public health priorities
