Responsibilities
- Guide members through the online program and deliver high-quality self-management education
- Leverage an online platform with a detailed curriculum and technology to help members track their progress
- Monitor blood glucose, blood pressure, labs, and health maintenance appointments
- Strategically counsel members through the Omada program using data science to inform outreach during critical moments
- Serve as a leader in cardiometabolic care within a digital care coaching practice through direct member care
Requirements
- Fluent in Spanish
- Active, unrestricted Certified Diabetes Care & Education Specialist (CDCES) certification through the Certification Board for Diabetes Care and Education (CBDCE)
- Minimum of 1 year professional experience as a CDCES or at least 3 years of experience in the field of diabetes
- Active, unrestricted Registered Dietitian (RDN or RD) or Registered Nurse (RN) certification with 5+ years of experience
- Bachelor’s degree
- Knowledge of or experience with blood glucose interpretation utilizing CGM technology
- Available to work full-time
- Have stellar written and oral communication skills
- Are a fast learner and are able to adapt to a constantly changing environment
Nice to Have
- At least 1 year of practice with patients with type 1 diabetes
- Additional training in informatics or technology
- Experience in digital health or telehealth
