Shields Health Solutions is seeking a Clinical Pharmacist to join our team. In this remote role, you will provide integrated support to patients and physicians, developing personalized relationships to improve medication adherence and promote evidence-based, cost-effective care.
What You'll Do
- Review and interpret clinician's patient care notes for clinical data needed for prior authorization approvals and care plan recommendations.
- Develop disease and drug specific clinical protocols to maximize patient adherence.
- Monitor ongoing patient adherence across all sites and specialty disease states, with a focus on oncology.
- Evaluate EMR data to review drug interactions, labs, allergies, and prescriptions, communicating directly with MDs.
- Recommend alternative therapeutic drug regimens and disease specific treatments according to guidelines.
- Monitor therapies for side effects and recommend drug therapy changes to prescribers.
- Perform medication therapy management (MTM): review medication regimens, advise clinicians, document interventions, and assess patient adherence.
- Conduct clinical interventions including comprehensive pharmacist consultation, motivational interviewing, drug regimen optimization, and medication reconciliation.
- Monitor productivity and report weekly statistics for growth development.
- Oversee technician's daily activities and verify accuracy on prior authorizations and care utilization logs.
- Identify and resolve insurance related issues and apply financial assistance where applicable.
- Supervise communication of support personnel compliant with HIPAA rules and regulations.
- Maintain current registration and remote pharmacist activity to comply with all drug laws.
- Communicate directly with other health professionals regarding patient care.
- Develop your own knowledge through education programs and train pharmacy staff and health care professionals.
- Manage, organize, and update relevant data using database applications.
- Apply observations to operational issues to increase productivity, quality, and customer-service standards.
- Assist with research projects and peer reviewing articles as needed.
- Identify trends, resolve problems, recommend improvements, and implement change.
- Actively participate in process improvement initiatives.
What We're Looking For
- Doctor of Pharmacy (PharmD) or equivalent advanced degree.
- Licensed Pharmacist in Texas.
Nice to Have
- 1+ years of experience.
- Knowledge of complex/chronic disease states and therapies used within specialty pharmacy, including Oncology, Transplant, Hepatitis C, Infectious Disease, Multiple Sclerosis, Rheumatoid Arthritis, GI disorders, Cystic Fibrosis, Psoriasis, Hyperlipidemia, and various inflammatory conditions.
- Knowledge using EPIC/QS1/WILLOW software.
Technical Stack
- Microsoft Word, Excel, PowerPoint.
- Database and reporting tools.
- EPIC/QS1/WILLOW software (preferred).
