Responsibilities
- Provide medication therapy management for patients with chronic conditions

- Conduct comprehensive medication reviews and patient consultations
- Ensure compliance with state and federal pharmacy regulations
- Support transitions of care through discharge planning and follow-up
- Educate patients on proper medication use and potential side effects
- Maintain accurate and up-to-date patient records
- Participate in quality improvement initiatives
- Monitor drug interactions and recommend therapeutic alternatives
- Assist in the development of clinical protocols and guidelines
- Respond to drug information inquiries from healthcare providers
- Promote safe medication practices across care settings
- Track patient outcomes related to medication interventions
- Participate in staff training and pharmacy operations as needed
- Ensure proper handling and storage of pharmaceuticals
- Support accreditation and regulatory requirements
- Engage in ongoing professional development
- Utilize electronic health records and pharmacy systems efficiently
- Coordinate prior authorizations and insurance-related documentation
- Contribute to formulary management discussions
- Provide input on medication safety alerts
- Assist in inventory control and medication procurement
- Support sterile compounding when required
- Participate in patient outreach programs
- Maintain patient confidentiality in accordance with HIPAA
