DaVita, which means 'giving life', is seeking a Clinical Coordinator to deliver empathetic, compassionate care in an outpatient dialysis center in Louisville, Kentucky. In this essential role, you will build meaningful relationships with patients and oversee a group of patients with support from Patient Care Technicians (PCTs).
What You'll Do
- Coordinate ESRD patient care and services, including trending lab values and tracking metrics on vital signs, weight, and outcome management.
- Ensure the clinical team delivers the highest level of care by focusing on patient safety, comfort, and well-being.
- Build meaningful and long-term relationships with patients and their families in an intimate outpatient setting.
- Deliver care to patients who are often dealing with multiple co-morbidities which require unique treatment plans.
- Oversee a group of patients with the support of PCTs to conduct patient observations, measure stats, and manage machine set up.
- Work with your head, heart, and hands each day in a fast-paced environment.
- Commit to improving patient health through clinical goal setting and quality improvement initiatives.
What We're Looking For
- At least 6 months of dialysis experience.
- Current Registered Nurse (RN) license in the state of practice.
- Current CPR certification.
- At least 18 months of registered nursing experience.
- Dialysis Charge RN readiness approval by Clinical Nurse Manager or RN-licensed FA/GFA.
- Associates Degree in Nursing (A.D.N) from an accredited school of nursing required; a three-year diploma from an accredited diploma program may be substituted.
- Completed training approved by the medical director and the governing body as required by CMS guidelines prior to operating the water treatment system.
- Willingness, desire, and ability to supervise.
- Basic computer skills and proficiency in MS Word and Outlook.
- Flexible schedule and ability to work mornings, evenings, weekends, and holidays.
Nice to Have
- Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN).
- Intensive Care Unit (ICU), Critical Care Unit (CCU), Emergency Room (ER), or Medical Surgical (Med Surg) experience.
- Bachelor of Science in Nursing (B.S.N).
- Supervisory experience.

You will be part of a small, interdisciplinary team of clinicians where Staff RNs are central. You will report to a Clinical Nurse Manager or RN-licensed FA/GFA.
