Catholic Charities Community Services (CCCS) is looking for a committed professional to join its Case Management Department, which works to ensure everyone in our community has access to the essentials for a dignified life. Our department serves a diverse population, including immigrants, seniors, and families experiencing financial hardship and food insecurity.
What You'll Do
- Provide effective client engagement, assessment, service planning, and follow-up using a strengths-based and trauma-informed approach.
- Deliver crisis intervention for emergencies related to shelter, food, clothing, and other basic needs through financial assistance, pantry access, and resource advocacy.
- Develop a comprehensive understanding of complex social service systems and partner with clients to navigate these networks effectively.
- Provide services to entire families, including parents, children, and grandparents, while identifying and building upon existing support systems.
- Conduct community outreach to help develop programs that build local capacity.
- Maintain collaborative relationships with community agencies, other Catholic Charities departments, and Archdiocesan partners.
- Enter all client and service information accurately into required Data Management Systems.
- Meet program deliverables in a timely and efficient manner while working closely with the funding partner, the Department of Youth and Community Development (DYCD).
- Complete requests for financial assistance and gather all necessary supportive documentation thoroughly and promptly.
- Attend and actively participate in weekly supervision, monthly department meetings, team meetings, community meetings, trainings, and other agency activities.
- Assist at other program sites as operational needs require.

- Proficiency in using a strengths-based and client-centered approach to service delivery.
- Strong skills in crisis intervention, assessment, and service coordination.
- Ability to navigate complex social service systems and advocate for client needs.
- Experience working with diverse populations, including immigrants, seniors, and families.
- Excellent organizational skills and the ability to manage program deliverables and documentation efficiently.
- Strong collaborative skills to work effectively with internal teams and external community partners.
- Commitment to attending regular supervision, meetings, and ongoing professional development.
- Flexibility to provide support at multiple service locations as needed.

Benefits & Compensation
- This position offers an hourly rate of $28.29.
- This is a non-exempt position based on a 35-hour work week, equating to an annual salary of $51,495.00.
Work Mode
This role requires onsite work at our location at 125 East 105th Street, New York, NY, 10029.
