Luminis Health is seeking a Case Manager to coordinate care for patients, ensuring safe and timely transitions across the healthcare continuum. Working under the clinical director of care management, you will identify, assess, plan, implement, and evaluate services to meet individual health needs, including social determinants of health.
What You'll Do
- Identify and prioritize patients in need of care management services using a holistic, multidisciplinary approach.
- Implement strategies such as motivational interviewing to promote patient engagement, self-care, and treatment adherence.
- Utilize evidence-based guidelines to verify appropriate level of care, enter clinical review in Epic, and document actions to avoid denied days.
- Manage observation stay patients assertively to ensure timely testing, treatment, and conversion to inpatient status or discharge.
- Develop and coordinate transition plans for patients discharged to home health, hospice, palliative care, or post-acute providers.
- Maintain clear and concise documentation in each patient record reflecting patient and family needs and support systems.
- Participate in nursing unit and department clinical outcome projects and process improvement initiatives.
- Identify patient situations requiring referral to other members of the health care team and adjust the plan of care as needed.
- Utilize risk and predictive analytic tools to apply tailored interventions to mitigate barriers, prolonged hospitalization, and readmissions.
- Maintain compliance with all regulatory standards (CMS, commercial insurers).
What We're Looking For
- BSN or ADN with equivalent experience (BSN must be achieved within 5 years of start date).
- Three years of experience in a clinical setting, ambulatory or post-acute.
- Current licensure as a registered nurse by the Maryland Board of Nursing.
