About the Role
The Case Manager will oversee service delivery for individuals with developmental and intellectual disabilities, ensuring compliance with care plans, coordinating with support staff, and maintaining accurate documentation.
Responsibilities
- Monitor and manage individual service plans
- Conduct regular client visits and assessments
- Ensure compliance with state and federal regulations
- Coordinate care with healthcare providers and families
- Maintain up-to-date case files and records
- Facilitate team meetings and service reviews
- Support transition planning for clients
- Track client progress toward goals
- Respond to incidents and report as required
- Assist in developing personalized support strategies
- Ensure adherence to agency policies
- Communicate regularly with guardians and families
- Supervise aspects of direct support staff activities
- Complete required documentation within deadlines
- Participate in quality assurance reviews
- Attend training sessions and professional development
- Promote client independence and community integration
- Manage crisis situations with appropriate protocols
- Ensure health and safety standards are met
- Coordinate referrals to external services
