What You'll Do
Guide members through personalized care coordination by evaluating their healthcare needs and eligibility using clinical tools and data. Develop strategies to address member challenges by reviewing benefit plans and identifying suitable internal and external support programs. Identify risk factors that could affect health outcomes and adjust care plans accordingly.
Collaborate with clinical teams to refer members to appropriate interventions when necessary, including crisis or specialized case management. Implement and track progress on care plans, ensuring alignment with member goals. Engage in case reviews with multidisciplinary teams to refine approaches and improve results.
Advocate for members by navigating healthcare systems and negotiating access to services that meet their needs. Use motivational techniques to encourage active participation in treatment and promote long-term health improvements. Educate members to make informed decisions alongside their providers and support independent management of their care.
Requirements
You must have at least two years of experience in behavioral health, social services, or a related human services field, along with two years of direct case management experience. Residency in Rockford, IL is required, and you must hold a valid Illinois driver’s license. Reliable transportation is essential, as 50–75% of your time will be spent traveling locally to meet members face to face. You should also be comfortable using standard documentation tools and following regulatory guidelines in care delivery.
Preferred Qualifications
Experience in discharge planning, managed care environments, or proficiency with Microsoft Office applications may enhance your candidacy.

Compensation
The base pay range for this position is $21.10 to $44.99 per hour, or equivalent annual salary, depending on experience, education, and local market factors. Actual offers may vary. You may also qualify for bonus, commission, or short-term incentive programs.
Work Mode
This is a local position based in Rockford, IL, requiring regular travel within the region. You must be able to meet members in person across the surrounding areas, with mileage reimbursement provided.
