Remote position - Requires Washington State Residency.​ RN strongly preferred. Position Purpose: As a care management professional, you'll assess, plan, and coordinate medical and supporting services across the care continuum for post-discharge members, promoting quality and cost-effective care. You'll complete medication reviews and work closely with teams to identify transition support services. Key Responsibilities: • Evaluate member needs by completing comprehensive post-discharge assessments • Perform medication reconciliation between pre-admit and post-discharge medications • Develop personalized care/service plans in collaboration with discharge planners, providers, specialists, and interdisciplinary teams • Assess current health status, resource needs, services, and treatment plans • Facilitate smooth transitions into active care management • Provide education and resource materials to members, caregivers, and providers to promote wellness • Coordinate services between PCPs, specialists, medical and non-medical resources • Conduct telephonic, digital, home, and site outreach to assess member needs • Document and maintain member information and care management activities • Ensure compliance with state, federal, and third-party payer regulations • Offer leadership feedback on care and quality delivery improvements • Perform other duties as assigned by team leaders • Comply with all organizational policies and standards Remote position - Requires Washington State Residency.​ RN strongly preferred. Education/Experience: Requires a Master's degree in Behavioral Health or Social Work, or a Degree from an Accredited School of Nursing with 2 – 4 years of related experience. License/Certification: • LISW, LCSW, LMSW, LMFT, LMHC, LPC, or RN required Pay Range: $55,100.00 - $99,000.00 per year Centene offers a comprehensive benefits package including: competitive pay, health insurance, 401K and stock purchase plans, tuition reimbursement, paid time off plus holidays, and a flexible approach to work with remote, hybrid, field or office work schedules. Actual pay will be adjusted based on an individual's skills, experience, education, and other job-related factors permitted by law, including full-time or part-time status. Total compensation may also include additional forms of incentives. Benefits may be subject to program eligibility.
