About the Role

Centene is hiring a Care Manager to develop, assess, and facilitate complex care management activities for members with primarily mental and behavioral health needs. You'll focus on creating personalized care plans and providing education to members and their families, contributing to the company's mission of transforming community health.

What You'll Do

  • Evaluate member needs via phone or in-home visits related to available resources and recommend or facilitate the care plan.
  • Perform telephonic, digital, home, and/or other site visits outreach to assess member needs and collaborate with resources.
  • Develop ongoing care plans for members with high-level acuity and identify needed providers, specialists, and community resources for mental health and substance use disorders.
  • Coordinate between the member, their family or caregivers, community resources, and the care provider team to ensure accessible services.
  • Monitor care plans and member status for changes and provide recommendations based on identified needs.
  • Facilitate care coordination and collaborate with appropriate providers or specialists to ensure timely access to care.
  • Collect, document, and maintain member information and care management activities to ensure regulatory compliance.
  • Provide education to members and their families on procedures, provider instructions, treatment options, referrals, and healthcare benefits.
  • Provide feedback to leadership on opportunities to improve and enhance care and quality delivery in a cost-effective manner.

What We're Looking For

  • Active Washington (WA) Clinical License: LCSW, LMSW, LMFT, LMHC, LPC, or RN.
  • Master's degree in Behavioral Health, Social Work, or a Degree from an Accredited School of Nursing.
  • 2–4 years of related experience.
  • Licensed Master's Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN with behavioral health experience as required by state contract.
  • Ability to work a standard schedule: Monday–Friday, 8:00 AM–5:00 PM Pacific Time.
  • Must be able to work Pacific Time (PT) hours, regardless of local time zone.
  • Experience managing or supporting behavioral health and care services for foster youth ages 0–26.
  • Ability to manage inbound and outbound calls, coordinate with hospitals, and support care transitions.
  • Strong ability to support crisis interventions and navigate complex cases.
  • Strong crisis management, engagement, and communication skills.

  • Salary range: $56,200.00 - $101,000.00 per year.

Work Mode
