Maximus Health is seeking a Care Coordinator to play an instrumental role in delivering a seamless care experience for our patients. In this remote position, you will be a key contributor to our patient concierge team, focusing on efficient coordination and empathetic support.
What You'll Do
- Coordinate and maintain provider schedules for video consultations, ensuring efficient appointment booking and minimizing conflicts.
- Monitor upcoming telehealth appointments, confirm availability, and troubleshoot technical issues.
- Liaise with laboratory partners to manage test orders, track results, and ensure timely communication of outcomes.
- Collaborate with pharmacy partners to facilitate prescription orders, refills, and address medication-related inquiries.
- Oversee and triage patient messages in digital platforms, ensuring prompt and appropriate responses.
- Escalate urgent or complex issues to the appropriate care team members while keeping patients informed.
- Provide friendly and empathetic support to patients, answering questions related to appointments, lab tests, prescriptions, and follow-ups.
- Educate patients on telehealth platform use and troubleshoot basic technical issues.
- Maintain accurate electronic health records, ensuring data integrity and confidentiality.
- Identify opportunities to streamline workflows and enhance patient experiences, bringing recommendations to leadership.
- Ensure compliance with all relevant healthcare regulations and company policies, including HIPAA.
- Participate in team meetings to review feedback, address operational challenges, and discuss quality improvement.
What We're Looking For
- 1–3 years of experience in a care coordinator, healthcare administration, or telehealth support role.
- Familiarity with EHR systems, telehealth platforms, scheduling software, and basic troubleshooting of technical issues.
- Excellent verbal and written communication skills to coordinate with patients, providers, and partners.
- Strong attention to detail and ability to manage multiple tasks efficiently in a fast-paced, digital environment.
- Empathetic, patient-focused approach with a commitment to delivering high-quality care and exceptional experiences.
- Understanding of healthcare regulations, especially HIPAA and data privacy guidelines.

- Associate’s or Bachelor’s degree in Healthcare Administration, Public Health, or a related field.
