As a Care Coordinator, you’ll support individuals managing complex chronic conditions by guiding them through care pathways and connecting them with essential services. Working remotely within a collaborative healthcare team, you’ll help patients access programs, schedule provider appointments, and navigate support systems to improve health outcomes.
What You'll Do
You’ll serve as a central point of contact for patients and providers, making and responding to calls to ensure timely communication. You’ll carry out non-clinical assessments under managerial guidance and escalate concerns to clinical staff when necessary. By coordinating tasks that move patients through their care journey, you’ll help maintain continuity and engagement across services.
You’ll also identify available resources for eligible members and work with nurses, dialysis centers, health plans, and community partners to facilitate enrollment. Accurate, confidential documentation is essential, and you’ll be expected to communicate clearly and professionally in all interactions.
Requirements
- High school diploma or GED
- Minimum of two years in a professional office setting
- Proven ability to manage sensitive information with discretion
- Strong verbal communication and interpersonal skills
- Proficiency in Microsoft Office, Excel, and Outlook
- Reliable high-speed internet connection with a quiet, private workspace at home
- Ability to work independently while contributing to team goals
Preferred Qualifications
- Experience with Electronic Medical Records systems
- Background in a clinical medical office environment (1–3 years)
- Prior work as a CNA or Medical Assistant
- Experience in care coordination, referral management, or care management
- Familiarity with insurance payer processes
- Direct patient support or medical office experience

Work Mode
This is a fully remote position with a full-time schedule from Monday to Friday. Work hours begin between 7:00 and 8:00 AM daily, with flexibility to accommodate patient needs. Nights, weekends, and on-call shifts are not required. A stable home office environment with high-speed internet (Ethernet connection may be required) is mandatory.
