About the Role
The Patient Care Coordinator will be responsible for managing patient care, coordinating schedules, and ensuring effective communication between healthcare providers and patients. This role involves a high level of organization and attention to detail to ensure that patients receive the best possible care.
Responsibilities
- Coordinate patient care plans and services
- Schedule and manage patient appointments
- Facilitate communication between patients and healthcare providers
- Maintain accurate and up-to-date patient records
- Assist with patient intake and discharge processes
- Collaborate with healthcare team to ensure patient needs are met
- Provide patient education and support
- Coordinate referrals to specialists and other services
- Monitor patient progress and update care plans as needed
- Ensure compliance with healthcare regulations and standards
- Manage patient follow-up care and appointments
- Assist with patient billing and insurance processes
- Coordinate transportation and other logistical needs for patients
- Provide emotional support and advocacy for patients
- Maintain confidentiality and privacy of patient information
- Assist with patient discharge planning and follow-up
- Coordinate care with community resources and support services
- Provide administrative support to healthcare team
- Assist with patient assessments and evaluations
- Monitor patient satisfaction and address concerns
- Coordinate care transitions between different healthcare settings
- Assist with patient and family education on health management
- Ensure timely and accurate communication with patients and providers
- Provide support to patients and families during difficult times
- Assist with patient care coordination and case management
- Coordinate care with other healthcare professionals and agencies
- Provide support to patients and families during transitions of care

- Bachelor's degree in healthcare administration or a related field
- Certification in healthcare or a related field
- Experience with telehealth and virtual care
- Bilingual or multilingual skills
