West Virginia University Health System is hiring a CAP RN Case Manager to play a vital role in supporting individuals with intellectual, developmental disabilities, mental health, and substance use challenges. You will work collaboratively with families and healthcare providers to ensure health, safety, and well-being through person-centered care planning and service coordination, all within a supportive and mission-driven environment.
What You'll Do
- Conduct comprehensive initial assessments of beneficiaries across medical, psychological, behavioral, financial, social, and other key areas.
- Develop, maintain, and update individualized, person-centered care plans, including emergency and disaster planning.
- Educate beneficiaries and primary caregivers about waiver services, Medicaid, and community resources.
- Coordinate services like personal care, home modifications, and waiver supplies in alignment with care plans.
- Monitor service quality and beneficiary progress through monthly phone calls and home visits.
- Collaborate with a multidisciplinary team to ensure a comprehensive approach to care.
- Evaluate care plan effectiveness and recommend changes to achieve desired outcomes.
- Provide emotional support and counseling to strengthen family support systems.
- Assist families in navigating challenges related to long-term care, behavioral issues, and complex medical needs.
- Maintain accurate, up-to-date participant records in compliance with Medicaid and program requirements.
- Ensure proper billing codes and documentation for case management, re-certifications, and supply billing.
- Act as an advocate for participants to secure necessary resources and services.
- Complete state-mandated and agency-required continuing education annually, including an NC Medicaid-certified training program within 90 days of employment.
- Stay current on CAP guidelines and best practices to enhance service delivery.
What We're Looking For
- Current North Carolina Registered Nurse (RN) license with a two-year or four-year degree.
- One year of case management experience in home care, long-term care, personal care, or a related field.
- Experience working with medically fragile children in pediatric healthcare settings and physically disabled adults with complex care needs.
- Strong understanding of Medicaid programs and compliance requirements.
- Excellent communication and interpersonal skills for working with families and multidisciplinary teams.
- Ability to manage multiple priorities and maintain detailed records.
- Completion of an NC Medicaid-certified training program within 90 calendar days of employment.
