About the Role

Role Overview

As a CAP Nurse, you will play a key role in supporting children with complex medical needs who are at risk of institutionalization. Your focus will be on enabling these children to remain safely in their homes by delivering essential nursing and case management services across Buncombe and Madison Counties.

Key Responsibilities

  • Verify eligibility and gather comprehensive client health and social histories
  • Conduct initial and ongoing assessments to identify care needs
  • Collaborate with families, schools, healthcare providers, and community agencies to support each child’s well-being
  • Develop individualized service plans and clearly communicate available resources and programs
  • Coordinate care delivery with a registered nurse and social worker as part of an interdisciplinary team
  • Refer clients to external services when additional support is needed
  • Maintain accurate documentation and evaluate program effectiveness over time
  • Conduct interviews and home visits to assess living conditions and care environments

Qualifications

Applicants must be graduates of an accredited nursing program and hold a valid North Carolina nursing license. A current NC driver’s license and clean driving record are required, along with personal auto insurance if using a personal vehicle for client transport. Candidates must also pass criminal background and drug screenings.

Preferred applicants will have at least one year of nursing experience and active RN certification.

Work Environment

This position operates locally within Buncombe and Madison Counties, with flexible scheduling to meet client needs. Work is conducted both in the field and through coordination with care partners.
