Role Overview
This remote position supports individuals in navigating their healthcare journey by evaluating needs, coordinating services, and promoting informed decision-making. The Care Coordinator works closely with members to understand eligibility, benefits, and social determinants of health that influence outcomes.
Key Responsibilities
- Perform in-depth assessments of member needs using data review and care management tools
- Develop strategies for care resolution based on benefit plans and available programs
- Recognize risk factors and service gaps that may affect health outcomes
- Refer complex cases to clinical teams or crisis resources when necessary
- Implement and track progress on individualized care plans
- Collaborate with healthcare professionals to address barriers and support goal achievement
- Present cases in multidisciplinary forums to ensure comprehensive review
- Report quality concerns through proper internal channels
- Advocate for members to secure appropriate services and benefit access
- Apply motivational techniques to encourage engagement and healthy behavior changes
- Educate members to support active participation in care decisions
- Ensure all activities follow regulatory standards and organizational policies
Qualifications
Required
- Minimum of two years in behavioral health or social services
- Proficiency with Microsoft Word, Excel, and Outlook
- Ability to work full-time hours from 8:00 AM to 5:00 PM in the assigned time zone
- Access to a private, distraction-free workspace
- Dependents must have separate care arrangements during work hours
Preferred
- Experience in case management or discharge planning
- Background in managed care environments
Work Environment
This is a fully remote role requiring a consistent, dedicated workspace. Schedules align with local market hours, and candidates must maintain uninterrupted focus during shifts.
Compensation & Benefits
Hourly pay ranges from $21.10 to $36.78, with eligibility for incentive programs.
