CVS Health / Oak Street Health is looking for an Analyst, Case Management – Transition of Care (Care Coordinator). In this role, you will utilize critical thinking to collaborate on the case management process, facilitating appropriate healthcare outcomes for members. You will assist with appointment scheduling, benefit access, and member education using care management tools.
What You'll Do
- Conduct comprehensive evaluations of member needs and eligibility, recommending approaches to case resolution.
- Identify high-risk factors and service needs, referring to clinical case management or crisis intervention as appropriate.
- Coordinate and implement assigned care plan activities while monitoring progress.
- Consult with case managers, supervisors, and Medical Directors using a holistic approach to overcome barriers.
- Identify and escalate quality of care issues through established channels.
- Utilize negotiation skills to secure appropriate options and services to meet member needs.
- Apply influencing and motivational interviewing skills to maximize member engagement and promote behavior change.
- Provide coaching and support to empower members in making independent medical and lifestyle choices.
- Help members actively and knowledgeably participate in healthcare decision-making with their providers.
- Utilize case management processes in compliance with regulatory guidelines and company policies.
What We're Looking For
- 2+ years of experience in behavioral health or social services.
- Ability to work 8:00 AM - 5:00 PM in the assigned market time zone.
- 2+ years of experience with Microsoft Office Applications (Word, Excel, Outlook).
- A dedicated workspace free of interruptions.
- Separate care arrangements for dependents during work hours.
Nice to Have
- Case management and discharge planning experience.
- Managed care experience.
