Shields Health Solutions is hiring an Ambulatory Clinical Pharmacist to provide integrated support to patients and physicians, improving the quality of patient care through personalized relationships. This remote role focuses on enhancing medication adherence and promoting evidence-based care by collaborating closely with an onsite clinical team.
What You'll Do
- Perform phone, virtual, or video-based assessments on disease state management and drug therapy education.
- Identify, resolve, and prevent medication-related problems.
- Provide medication management, including reconciliation, dose optimization, and adherence counseling.
- Collaborate with the interdisciplinary care team to develop individualized, evidence-based care plans.
- Monitor and evaluate medication regimens for therapeutic goals, adverse effects, and drug interactions.
- Identify opportunities for drug therapy optimization and recommend medication cost savings.
- Participate in quality improvement initiatives, clinical outcome studies, REMs compliance, and process improvement.
- Optimize resources by providing clinical justification for complex prior authorizations and appeals.
- Collaborate with healthcare providers to ensure seamless medication management and care coordination.
- Support occasional peer-to-peer requests upon demand.
- Perform clinical assessments and educational counseling at the drug-level for patients on targeted medications.
- Ensure all clinical criteria comply with FDA guidance, clinical guidelines, and evidence-based literature.
- Assist with workflow optimization initiatives to enhance efficiency, productivity, and quality of care.
- Advocate for patient safety and medication literacy through education and counseling.
- Protect organization's value by keeping information confidential.
- Update job knowledge through educational opportunities, professional publications, and networking.
- Attend team meetings, conferences, and professional development activities.
- Stay updated with the latest advancements in therapy management and incorporate them into practice.
- Perform other duties as assigned.
What We're Looking For
- Hold a Doctor of Pharmacy (PharmD) degree.
- Be a licensed pharmacist in California.
- Be an advanced practice pharmacist in California by meeting two of three specified criteria.
Nice to Have
- Completion of PGY1/PGY2 residency.
Team & Environment
You will collaborate closely with a hybrid Ambulatory Clinical Pharmacist team onsite and the broader interdisciplinary care team, reporting to the Senior Director of Clinical Services and Manager of Clinical Services.
Benefits & Compensation
- Salary range: $120,000 to $128,000 plus bonus pursuant to the terms of any bonus plan if applicable.
