Oak Street Health is seeking an A1A Case Manager RN to join our team. In this role, you will be responsible for telephonically and face-to-face assessing, planning, implementing, and coordinating all case management activities to evaluate medical needs and facilitate member wellness. This position is a key part of the Aetna One Advocacy (A1A) program, Aetna's premier service and clinical offering.

What You'll Do

Develop proactive plans to address issues and enhance short and long-term outcomes for member wellness.
Conduct evaluations of member needs and benefit plan eligibility using clinical tools and data review.
Apply clinical judgment to incorporate strategies designed to reduce risk factors and address complex health and social indicators.
Assess information from various sources to address all conditions, including co-morbid and multiple diagnoses impacting functionality.
Review prior claims to address potential impact on current case management and eligibility.
Assess a member's level of work capacity and related restrictions or limitations.
Using a holistic approach, assess the need for referral to clinical resources for assistance in determining functionality.
Consult with supervisor and others to overcome barriers and present cases at case conferences for multidisciplinary focus.
Utilize case management processes in compliance with regulatory and company policies and procedures.
Utilize interviewing skills to ensure maximum member engagement and discern health status and needs.

What We're Looking For

A Registered Nurse with an unrestricted license in state of residence, preferably with multi-state/compact privileges.
Willingness and ability to be licensed in all non-compact states; upon hire, required to obtain all 50 state licenses.
3+ years of clinical practice experience as an RN.
Must live within a commutable distance of High Point, NC (typically within 45 minutes maximum).
Associates Degree in Nursing required.

Nice to Have

Case Management experience in an integrated model.
Certified Case Manager (CCM) certification.
BSN preferred.
