Oak Street Health is seeking an A1A Case Manager RN to join our team. In this role, you will be responsible for telephonically and face-to-face assessing, planning, implementing, and coordinating all case management activities to evaluate medical needs and facilitate member wellness. This position is a key part of the Aetna One Advocacy (A1A) program, Aetna's premier service and clinical offering.
What You'll Do
- Develop proactive plans to address issues and enhance short and long-term outcomes for member wellness.
- Conduct evaluations of member needs and benefit plan eligibility using clinical tools and data review.
- Apply clinical judgment to incorporate strategies designed to reduce risk factors and address complex health and social indicators.
- Assess information from various sources to address all conditions, including co-morbid and multiple diagnoses impacting functionality.
- Review prior claims to address potential impact on current case management and eligibility.
- Assess a member's level of work capacity and related restrictions or limitations.
- Using a holistic approach, assess the need for referral to clinical resources for assistance in determining functionality.
- Consult with supervisor and others to overcome barriers and present cases at case conferences for multidisciplinary focus.
- Utilize case management processes in compliance with regulatory and company policies and procedures.
- Utilize interviewing skills to ensure maximum member engagement and discern health status and needs.
What We're Looking For
- A Registered Nurse with an unrestricted license in state of residence, preferably with multi-state/compact privileges.
- Willingness and ability to be licensed in all non-compact states; upon hire, required to obtain all 50 state licenses.
- 3+ years of clinical practice experience as an RN.
- Must live within a commutable distance of High Point, NC (typically within 45 minutes maximum).
- Associates Degree in Nursing required.
Nice to Have
- Case Management experience in an integrated model.
- Certified Case Manager (CCM) certification.
- BSN preferred.
