Role Description:
As a Case Management Nurse I, you will serve as a key advisor who actively engages with members through various channels to support their complex healthcare needs. You will create personalized care plans ensuring optimal health outcomes by partnering with members, families, healthcare providers, and community resources.

You are a dedicated healthcare professional motivated to provide personalized support and education. You excel in fast-paced environments, demonstrating strong time management, prioritization, and multi-tasking skills. You are resourceful, collaborative, adaptable, and technologically proficient.
Preferred Candidates:
Located in specific states, with prior health plan experience and diverse clinical background, including behavioral health expertise.
Certification Requirement:
Must obtain Certified Case Manager (CCM) certification within 24 months of hire.
Preferred Qualifications:
- Certified Case Manager (CCM)
- Care navigation/coordination experience
- Behavioral health experience
Required Qualifications:
- Accredited nursing program completion
- Active, unrestricted RN License
- 4+ years diverse clinical experience
- Strong communication skills
- Professional demeanor
- Critical thinking abilities
- Technical proficiency
- Understanding of healthcare regulations
Key Responsibilities:
- Utilize critical thinking for complex condition identification
- Design individualized care plans
