Mindoula Health is hiring a WA Licensed Therapist (LMFT, LCSW, LMHC) for our Substance Use Disorder Program. In this 100% remote role, you will provide specialized care to substance-exposed members in Washington State, focusing on prenatal and postpartum care within our family-focused culture. You will have flexibility to create therapeutic groups and utilize your preferred evidence-based practices.
What You'll Do
- Assess, plan, and implement individualized care strategies directed toward the most appropriate level of care.
- Collaborate with members, families, and healthcare providers to develop individualized care plans.
- Conduct individual counseling and group therapy sessions with adolescents and adults.
- Identify and initiate referrals for social service programs, including financial, psycho-social, community, and state supportive services.
- Advocate for members and families to ensure their needs and choices are fully represented.
- Utilize approved clinical criteria to assess and determine appropriate levels of care.
- Document all member assessments, care plans, and referrals.
- Be responsible for achieving set goals and Key Performance Indicators (KPIs).
- Learn and apply the StrongWell model, taking ownership of outcome-based care.
- Participate in interdisciplinary team meetings and utilization management rounds.
- Promote responsible and ethical stewardship of company resources.
- Maintain excellent punctuality and attendance.
What We're Looking For
- Current LCSW, LMFT, or LMHC license in Washington State.
- Clinical experience working with adults and adolescents.
- Familiarity with Medicare and Medicaid procedures.
- Proven remote work experience.
Nice to Have
- Experience with a substance abuse population.
- Background in maternal substance abuse.
