Responsibilities
- Deliver direct clinical care via telehealth to patients with neurological conditions, memory disorders, or dementia
- Perform patient assessments, create care plans, and oversee ongoing treatment in coordination with supervising physicians, including prescribing therapies
- Collaborate with our neurologist(s) and wider care team to maintain continuity of care
- Educate patients and their families about diagnoses, treatment plans, and disease progression
- Keep accurate and timely clinical documentation in the EMR in line with regulatory standards
Requirements
- Active, unrestricted Nurse Practitioner license in Massachusetts
- At least 2 years of clinical experience in neurology, memory care, or dementia care settings
- Strong clinical judgment and clinical documentation skills
- Comfortable managing a full patient panel independently with physician collaboration
- Excellent communication skills when interacting with patients experiencing cognitive decline and their caregivers
Nice to Have
- Certification in geriatrics, adult-gerontology, or family practice preferred
