Work as a contracted LPN in Virginia to support a remote patient monitoring program focused on early detection of diabetic foot complications. You'll use a specialized in-home imaging scale to review patient data and help guide timely clinical interventions—all from a fully remote setting.
What You'll Do
- Review daily foot images and weight measurements collected through a smart scale and accessed via a secure web portal
- Identify and report urgent findings to physicians or notify patients when follow-up is needed
- Conduct scheduled check-ins with patients, starting with a caseload of up to 50 individuals
- Deliver adherence reminders and respond to patient inquiries about device use or health changes
- Document all interactions thoroughly and accurately in the system
- Collaborate with physicians and internal teams to ensure continuity of care
- Escalate technical or clinical issues to the appropriate team members when necessary
Requirements
- Current Licensed Practical Nurse (LPN) credential valid in Virginia
- Residence within the United States
- Experience caring for patients with diabetes
- Proficiency with digital tools, including cloud-based platforms and online portals
- Ability to install apps, convert files (e.g., Word to PDF), and upload documents to web systems
- Strong written and verbal English communication skills
- Capacity to quickly learn and apply new clinical protocols
- Dedication to expanding access to effective, patient-centered care
