Monitor patient health remotely as a Licensed Practical Nurse in Virginia, supporting a growing remote care initiative focused on diabetes management. You'll review daily data from foot imaging and weight measurements collected via medical devices, using a secure web portal to track changes and identify concerns.
Key Responsibilities
- Review patient data daily, including foot images and weight trends, and flag urgent findings to physicians or patients
- Conduct scheduled check-ins with patients, starting with a caseload of up to 50 individuals
- Provide timely follow-up for patients with health changes, technical issues, or questions about their care
- Document all interactions accurately and ensure patient adherence to monitoring protocols
- Collaborate with physicians and coordinate with clinical teams to support patient needs
- Assist patients with device use and troubleshoot basic technical issues
Qualifications
Candidates must be licensed as an LPN in Virginia and comfortable using digital tools beyond standard office software. This includes file conversion, cloud platforms, app installation, and secure document uploads. Strong written and verbal English communication skills are essential, along with experience in diabetes care.
Preferred applicants have a compact license, background in diabetic foot complications, telehealth experience, or multilingual abilities. Familiarity with clinical research or Good Clinical Practice (GCP) is also valued.
