Pennant Services is hiring a Triage RN to join our remote team supporting Signature Healthcare at Home. In this role, you'll deliver professional nursing care coordination via telephone, evaluating patient health concerns and providing advice for home health, hospice, and primary care patients.
What You'll Do
- Manage nurse-directed patient telephone calls.
- Evaluate and assess patient’s health care needs based on signs and symptoms utilizing established triage protocols and guidelines.
- Obtain appropriate input and direction from the provider as needed to determine the best clinical course of action.
- Document accurately the symptoms/complaints, nursing assessments, advice provided, and patient response.
- Follow policies, procedures, and protocols to ensure consistency and improve health care outcomes.
- Coordinate and schedule same-day appointments on provider schedules.
- Utilize standing orders, triage protocols and algorithms appropriately.
- Manage prescription refills per protocol.
- Provide and support patient care instructions and education.
- Assure pertinent medical information is forwarded to proper departments for continuity of care.
- Document in the electronic health record (EHR) the triage assessment, plan, implementation, evaluation and follow-up.
- Maintain call rate in keeping with productivity standards.
- Maintain a positive attitude and work collaboratively with all staff members.
- Support the mission, values and goals of Signature.
What We're Looking For
- Must possess a nursing degree from an accredited college or university.
- Must possess a current, unencumbered, active license to practice as an RN in multiple states.
- Current CPR Certification.
Nice to Have
- Bachelor’s Degree preferred.
- 1-2 years’ recent experience in home health, hospice, home care, and acute care highly preferred.
- Prior experience supervising nurses, nursing assistants and care aides preferred.
