Clarvida Careers is hiring a Tennessee Health Link Lead (Program Lead) to assist in the oversight of our program under the supervision of the Regional Program Director or Assistant Director. In this role, you will ensure client-centered care and case coordination is provided competently and ethically.
What You'll Do
- Assess clients on intake as needed, complete on-call duties, and ensure Clarvida approved behavioral health therapeutic services are provided.
- Work with Care Coordinators to ensure all assigned clients' treatment teams collaborate for quality care.
- Ensure Care Plans and documentation are consistent with Clarvida policy, state licensing regulations, and COA accreditation requirements.
- Ensure all clinical documentation is completed within established time frames and rectify deficits; monitor monthly and quarterly reports.
- Monitor and work with Care Coordinators to close gaps in care (HEDIS), tracking progress weekly.
- Ensure/conduct weekly supervision with Care Coordinators according to contract and COA requirements.
- Facilitate Treatment Team meetings and review presentations on each assigned client as required.
- Attend all quarterly director meetings and regional leadership meetings.
- Support fellow team members to ensure the best outcomes for all clients and families.
- Contribute to the Performance Improvement Cycle, including data collection to improve services and documentation.
- Ensure PQI activities are completed accurately and timely, including Peer and PQI audits, and participate in MCO evaluations.
- Manage and/or assist leadership team in overseeing client census and productivity standards/requirements for Care Coordinators.
- Assist in program development, networking, and collaboration between program areas.
What We're Looking For
- Bachelor’s degree in a Human Service discipline from an accredited four-year college or university.
- At least 3 years of relevant experience in the Mental Health field.
- Be a licensed driver with automobile insurance in good standing and access to reliable transportation.
Nice to Have
- Experience working with children/adolescents in a therapeutic, community-based treatment environment.
