Dungarvin is hiring a Supportive Housing Case Manager to empower individuals through supportive housing programs. You will provide ongoing life skills development and targeted interventions to prevent eviction, directly impacting client independence and stability.
What You'll Do
- Conduct assessments and interviews with clients to determine eligibility.
- Develop and update action plans, case notes, and other client file documents.
- Provide ongoing life skills development to improve client independence and quality of life.
- Deliver specialized interventions when clients are at risk of eviction.
- Connect clients to medical, mental health, and other supportive counseling services.
- Establish a network of community services and conduct appropriate referrals.
- Guide clients through application, qualification, and re-determination for SSI, SSDI, Medicaid, Medicare, SNAP, and other services.
- Occasionally conduct visits in the client’s home.
- Enlist the support of property management for wellness checks.
- Track and report outcomes and provide data updates as directed.
- Attend and actively participate in scheduled internal and external meetings.
- Travel throughout the area and provide transportation to clients as needed.
- Represent the organization at professional meetings and community events.
- Use assigned technology in the community, including laptop, cell phone, and designated software.
What We're Looking For
- A Bachelor's Degree in social services or a related field and a minimum of one year of experience coordinating supportive housing or independent living services in a social service setting
- OR a High School Diploma or GED and two years of relevant experience.
- A valid driver's license, insurance, and reliable transportation.
- Ability to perform CPR/First Aid as needed.
- Ability to lift up to 50 lbs.
- Must be at least 21 years of age.
