DaVita seeks a compassionate Social Worker to provide vital support and advocacy for our dialysis patients. In this role, you will help patients and their families navigate the significant challenges of End Stage Renal Disease, fostering resilience and improving health outcomes.
What You'll Do
- Build meaningful and long-term relationships with patients and their families in an intimate outpatient setting.
- Support and advocate for patients undergoing the difficult transition to life on dialysis.
- Help patients understand their rights and responsibilities.
- Guide patients in managing the demands of End Stage Renal Disease.
- Commit to improving patient health through clinical goal-setting and quality improvement initiatives.
What We're Looking For
- Master's degree in Social Work (MSW) with a specialization in clinical practice.
- Meets all state required regulations to practice in the dialysis setting.
- Demonstrated knowledge of government and private insurance programs.
- Basic computer skills in MS Word, Excel, PowerPoint, and Outlook.
- Functional proficiency with DaVita specific applications within 60 days.
