At EX Squared, we put children first, and we are looking for a dedicated Social Services Care Manager to join our mission. You will provide continuity of care, care coordination, and direct support to families of children with complex behavioral, medical, and social needs. This role focuses on psychoeducation, emotional support, navigating the healthcare system, and advocating for essential resources.
What You'll Do
- Provide continuity of care and coordinate services for patients and their families.
- Identify and solution for care gaps and service needs.
- Complete assessments with families and assist the interdisciplinary team in developing a plan of care that integrates patient and family preferences.
- Collaborate with patients and families to achieve service plan goals, develop life skills, and support financial stability.
- Create plans for high-stress treatment situations, including mental health crises, behavioral issues, and family conflict.
- Provide education on risk prevention to patients and families.
- Advocate for resources, work to remove barriers, and connect families with available support.
- Participate in scheduled consultations with an interdisciplinary team to monitor patient progress.
- Represent Imagine Pediatrics commendably to patients, families, providers, and the community.
- Conduct telehealth visits with patients and families.
What We're Looking For
- A Master’s degree with a major in social work (MSW) is required.
- A minimum of three (3) years of post-graduate experience in social work or a closely related field.
- A minimum of two (2) years of direct case management experience.
- Experience working with a pediatric population and family systems.
- Knowledge and application of motivational interviewing, goal setting, intervention, and needs assessment.
- Knowledge of social work principles, including crisis prevention and stabilization.
- Proficiency with Microsoft Office, Outlook, and the ability to work in online platforms.
- Willingness and ability to accommodate patient scheduling needs.
Nice to Have
- Experience with Child Protective Services (CPS) and foster care is highly preferred.
- Experience providing telehealth services is preferred.
- Bilingual proficiency in Spanish is strongly preferred.

You will work closely with an interdisciplinary team including MDs, APPs, RNs, CTAs, mental health therapists, community health navigators, pharmacists, and dietitians.
