ICF is hiring a Resource Specialist - REMOTE

Responsibilities

  • Evaluate referrals from case managers to link service recipients with Federally Qualified Health Centers (FQHCs), perform follow-up contacts, and collect necessary documentation to finalize referrals.
  • Perform eligibility assessments for services and input intake data into a tracking database.
  • Engage in outreach to identify new FQHCs, establish points of contact, assess available resources, and expand network capabilities.
  • Establish records for newly verified FQHCs in the case management system and coordinate with case management to enable provider invoicing.
  • Maintain accurate data entry to monitor client usage of FQHC services and ensure alignment with eligibility requirements.
  • Deliver regular updates to the case management team on referral statuses as needed.
  • Attend weekly meetings to assess FQHC placements, service utilization, and resolve access-related challenges.
  • Maintain communication with FQHCs to confirm service delivery and validate data for invoicing compliance.
  • Provide daily support for assigned tasks and project deliverables.
  • Assist in developing coordination tools and response protocols for FQHCs.
  • Support follow-up activities, including data input and reporting tasks.
  • Demonstrate flexibility by assisting team members when required.
  • Complete additional duties as directed by project leadership.

Compensation

The pay range for this position based on full-time employment is: $55,388.00 - $94,160.00 Nationwide Remote Office (US99)
