Chronic Care Staffing is hiring a Remote RPM Coordinator – CMA to play a vital role in supporting patients living with chronic or high-risk conditions. You will utilize Remote Patient Monitoring (RPM) to track patient health data, provide education, and act as a crucial liaison between patients and healthcare providers, with the goal of improving outcomes and reducing hospitalizations.
What You'll Do
- Conduct patient onboarding and device education calls for new RPM enrollees.
- Monitor daily biometric data, such as blood pressure, glucose, weight, and oxygen saturation.
- Identify abnormal readings and follow escalation protocols to notify providers.
- Perform monthly RPM engagement calls and address patient concerns.
- Troubleshoot device connectivity or usage issues.
- Communicate with patients and caregivers about their conditions, goals, and progress.
- Support patient self-management and adherence to provider-developed care plans.
- Coordinate with healthcare teams to ensure timely responses to alerts.
- Document all interactions and interventions accurately in the EHR/RPM portal.
- Educate patients on lifestyle modifications, risk reduction, and monitoring importance.
- Comply with CMS RPM billing and documentation requirements.
- Meet productivity expectations for monitoring and outreach.
- Represent Chronic Care Staffing professionally and compassionately at all times.
What We're Looking For
- Unencumbered active CMA or RMA certification.
- Active BLS certification.
- Ability to communicate clearly and compassionately via phone or messaging.
- Excellent documentation, time management, and critical thinking skills.
- Experience with EHR systems and cloud-based platforms.
- Familiarity with Google Suite.
- A HIPAA-compliant workspace free from distractions.
- A room with a lockable door to prevent accidental PHI exposure.
- High-speed internet connection.
- Use of a CCS-approved computer with dual monitors.
Nice to Have
- Strong understanding of Remote Patient Monitoring protocols and platforms.
- Proficiency in recognizing clinical red flags in biometric data.
