As a Remote Patient Monitoring Specialist, you'll play a key role in managing patient health outside traditional clinical settings. Using remote monitoring technology, you'll track vital signs such as blood pressure, oxygen levels, glucose readings, and weight, ensuring timely review and response to changes in patient status.
What You'll Do
- Continuously monitor incoming biometric data from patients using connected devices
- Evaluate alerts and identify deviations from baseline according to established clinical guidelines
- Reach out to patients via phone or video to assess symptoms, offer guidance, and reinforce care plans
- Provide education on proper device use and support ongoing engagement in remote care programs
- Work closely with physicians and care teams to adjust treatment strategies for chronic conditions
- Document patient interactions and clinical findings accurately in the Electronic Health Record (EHR)
- Determine when clinical concerns require urgent attention and initiate appropriate escalation
Requirements
- Active Registered Nurse (RN) license in good standing
- 1–3 years of hands-on clinical experience, ideally in telehealth, home health, or care coordination
- Proven ability to interpret patient vitals and clinical trends
- Experience with EHR systems and virtual care platforms
- Strong organizational, written, and verbal communication skills
- Ability to manage responsibilities independently in a remote work setting
- Reliable computer setup with a second monitor for efficient workflow
- Noise-cancelling headset and stable primary internet connection, with access to a backup connection
