Seasoned Recruitment is hiring a Remote Nurse Practitioner / Lead Nurse Practitioner to build and guide a new telehealth-based clinical team. You will lead efforts to manage complex Medicaid populations in Arizona, improving patient outcomes and reducing costs through coordinated, innovative care. This role is perfect for an NP seeking to practice autonomously in a dynamic startup environment while making a significant impact.
What You'll Do
- Serve as the operational lead for the interdisciplinary care team.
- Lead care coordination efforts for complex Medicaid populations.
- Practice autonomously with prescribing authority.
- Manage care delivery primarily through telehealth and remote coordination.
- Coordinate care across multiple providers, including primary care physicians and health plan case managers.
- Support proactive care management, including risk stratification and early detection of health declines.
- Provide 24/7 clinical access to help patients avoid unnecessary Emergency Room visits.
- Support patients during hospitalizations and post-discharge transitions.
- Help build and shape a new, innovative value-based care model.
What We're Looking For
- 1+ years of experience as a licensed Nurse Practitioner.
- Active/clear Nurse Practitioner license to practice in the State of Arizona.
- Active/Clear DEA.
- 1+ years of experience working in a rehab setting or rehabilitation facility centers.
- Strong knowledge in HIPAA compliance as it pertains to telehealth.

Benefits & Compensation
- Salary: $130,000 to $175,000
Work Mode
This is a fully remote position requiring candidates to have an active Arizona Nurse Practitioner license.
