CareHarmony is looking for a certified Medical Assistant (CCMA) to join our team as a Care Coordinator. In this role, you will leverage your clinical background to deliver value-based care management by working directly with providers to coordinate comprehensive care for chronic care management patients.
What You'll Do
- Manage a patient census with a resolution-driven approach to close clinical and non-clinical care gaps.
- Identify and coordinate community resources to benefit patient care.
- Provide patient education and improve health literacy for chronic condition management.
- Perform medication management, including reconciliation, adherence checks, and coordinating refills.
- Ensure timely delivery of services such as Home Health, DME, and Home Infusion.
- Resolve patient questions and create open dialogue to understand their needs.
- Assist in managing referrals and scheduling appointments.
What We're Looking For
- Active CCMA certification.
- At least 3 years of direct patient-facing work experience.
- Technical aptitude with the Microsoft Office Suite.
- Excellent written and verbal communication skills.
- Must have active high-speed Wi-Fi and a home office or HIPAA-compliant workspace.

- Compensation: $18/hr to start with the ability to earn up to $20/hr based on workload.
