We are seeking a dedicated Remote Care Coordinator with current CMA or RMA certification to provide personalized support for patients managing chronic health conditions. In this fully remote position, you'll play a key role in improving patient outcomes through consistent follow-up, education, and collaboration with care teams.
What You’ll Do
- Conduct scheduled monthly outreach calls to enroll and support patients in Chronic Care Management programs
- Complete health risk assessments and assist with transitions of care following hospital discharge
- Guide patients through Remote Patient Monitoring setup and ongoing use
- Review medications, care plans, and diagnoses with patients and their families
- Coordinate with physicians and clinical staff to ensure continuity of care
- Help patients connect with local services and support programs
- Document all interactions accurately in electronic health record systems
- Follow CMS guidelines and maintain compliance with quality reporting standards
What We Require
- Current, unrestricted CMA or RMA certification
- Valid BLS credential
- Residency in the Pacific or Mountain Standard Time zone in the U.S.
- Recent hands-on experience in clinical care, care coordination, or chronic care management
- Proficiency with EHR platforms and Google Suite applications
- Strong critical thinking and communication abilities
- Proven ability to work independently in a secure, HIPAA-compliant home office
Preferred Background
- Experience in transitional care, remote monitoring, or patient education
- Familiarity with community-based health resources
- Background in quality reporting or CMS compliance
- Additional training or certification in case or care management
- Success working remotely within a team-based clinical model
Compensation & Benefits
This full-time role offers a pay rate of $18.00 to $21.00 per hour, with no weekend shifts required.
