CareHarmony is hiring a Remote LPN Intake Coordinator to drive value-based care initiatives by working comprehensively with providers. In this role, you will conduct health assessments, coordinate resources, and provide vital education for chronic care management patients.
What You'll Do
- Accept transfers from the Patient Enrollment team to conduct preliminary health assessments for newly enrolled patients.
- Resolve patients' questions and create an open dialogue to understand their needs.
- Identify and coordinate community resources with patients to benefit their care.
- Provide patient education and improve health literacy on the management of chronic conditions.
- Assist with medication management, including identifying potential medication concerns, adherence issues, and coordinating refills.
- Assist in ensuring timely delivery of services to patients, such as Home Health, DME, and Home Infusion.
What We're Looking For
- Active Michigan & Compact/Multi-State license as an LPN/LVN
- At least 3 years of direct patient-facing work experience
- Technical aptitude with the Microsoft Office Suite
- Excellent written and verbal communication skills
- Ability to quickly adapt to a fast-paced work environment
- Must have active high-speed Wi-Fi
- Must have a home office or HIPAA-compliant workspace
Nice to Have
- Experience with Epic electronic health records
- Bilingual language skills
- Additional Single State licensures
