CareHarmony is hiring a Remote Licensed Practical Nurse (LPN/LVN with NLC) to join our team as a Care Coordinator. You will play a central role in delivering value-based care management by working comprehensively with providers and managing a dedicated patient census. Your focus will be a resolution-driven approach to closing gaps in both clinical and non-clinical patient care.
What You'll Do
- Manage a patient census with a resolution-driven approach to close gaps in clinical and non-clinical patient care.
- Identify and coordinate community resources that would benefit patient care.
- Provide patient education and improve health literacy on the management of chronic conditions.
- Perform medication management, including identifying potential concerns, reconciliation, adherence, and coordinating refills.
- Assist in ensuring timely delivery of services like Home Health, DME, and Home Infusion.
- Resolve patient questions and create an open dialogue to understand their needs.
- Assist and manage referrals and appointment scheduling.
What We're Looking For
- Active Multi-State/Compact License (LPN/NLC/LVN).
- At least 3 years of direct patient-facing work experience.
- Technical aptitude with the Microsoft Office Suite.
- Excellent written and verbal communication skills.
- Must have active high-speed Wi-Fi.
- Must have a home office or HIPAA-compliant workspace.
Nice to Have
- Experience with Epic EHR.
- Bilingual communication skills.
- Additional single state LPN licensures.

- Compensation starts at $21/hr with the ability to earn up to $28/hr based on production.
