CareHarmony was looking for a Remote - Licensed Practical Nurse (LPN) - Michigan Licensure

CareHarmony is looking for a Licensed Practical Nurse (LPN) with an active Michigan license. In this remote role, you will work comprehensively with providers to deliver value-based care management initiatives, focusing on managing a patient census, coordinating resources, and providing patient education.

What You'll Do

  • Manage patient census with a resolution-driven approach to close gaps in clinical and non-clinical care.
  • Identify and coordinate community resources to benefit patient care.
  • Provide patient education and health literacy on the management of chronic conditions.
  • Perform medication management, including identifying potential concerns, reconciliation, adherence, and coordinating refills.
  • Assist in ensuring timely delivery of services, including Home Health, DME, Home Infusion, and other critical needs.
  • Resolve patients' questions and create an open dialogue to understand their needs.
  • Assist and manage referrals and appointment scheduling.

What We're Looking For

  • Active Michigan LPN license.
  • At least 3 years of direct patient-facing work experience.
  • Technical aptitude with the Microsoft Office Suite.
  • Excellent written and verbal communication skills.
  • Must have active high-speed Wi-Fi.
  • Must have a home office or HIPAA-compliant workspace.

Nice to Have

  • Experience with Epic.
  • Bilingual language skills.

Benefits & Compensation

  • Compensation starts at $22/hour ($21/hr + $1/hr MI license bump) with the ability to earn up to $28/hour based on production.
