CareHarmony was looking for a Remote - Licensed Practical Nurse (LPN) - Massachusetts Licensure Required

CareHarmony is looking for a remote Licensed Practical Nurse (LPN) to join our care coordination team. In this role, you will work comprehensively with providers to deliver value-based care management initiatives. You will manage a patient census with a resolution-driven approach to close gaps in care, coordinate resources, and provide patient education.

What You'll Do

  • Manage a patient census with a resolution-driven approach to close gaps in clinical and non-clinical patient care.
  • Identify and coordinate community resources that would benefit your patients' care.
  • Provide patient education and health literacy on the management of chronic conditions.
  • Perform medication management, including identifying potential concerns, reconciliation, adherence, and coordinating refills.
  • Assist in ensuring timely delivery of services, including Home Health, DME, and Home Infusion.
  • Resolve patients' questions and create an open dialogue to understand their needs.
  • Assist and manage referrals and appointment scheduling.

What We're Looking For

  • Active Massachusetts LPN License
  • At least 3 years of direct patient-facing work experience
  • Technical aptitude with the Microsoft Office Suite
  • Excellent written and verbal communication skills
  • Must have active high-speed Wi-Fi
  • Must have a home office or HIPAA-compliant workspace

Nice to Have

  • Experience with Epic EHR
  • Bilingual skills
  • Additional state LPN licensures
