CareHarmony is seeking a Licensed Practical Nurse (LPN) with an active New York license to join our team as a Care Coordinator. In this remote role, you will work comprehensively with providers to deliver value-based care management initiatives, focusing on identifying resources and coordinating needs for chronic care management patients.
What You'll Do
- Manage a patient census with a resolution-driven approach to close gaps in clinical and non-clinical care.
- Identify and coordinate community resources that would benefit patient care.
- Provide patient education and health literacy on the management of chronic conditions.
- Perform medication management, including identifying concerns, reconciliation, adherence, and coordinating refills.
- Assist in ensuring timely delivery of services like Home Health, DME, and Home Infusion.
- Resolve patients' questions and create an open dialogue to understand their needs.
- Assist in managing referrals and appointment scheduling.
What We're Looking For
- Active New York LPN License.
- At least 3 years of direct patient-facing work experience.
- Technical aptitude with the Microsoft Office Suite.
- Excellent written and verbal communication skills.
- Must have active high-speed Wi-Fi.
- Must have a home office or HIPAA-compliant workspace.
Nice to Have
- Experience with Epic.
- Bilingual skills.
- Any additional state LPN licensures.

- Compensation starts at $22/hour ($21 base + $1 NY license bump) with the ability to earn up to $28/hour based on production.
