CareHarmony is hiring a Remote Licensed Practical Nurse (LPN / LVN) to work comprehensively with healthcare providers, delivering value-based care management initiatives. As a Care Coordinator, you will identify resources and coordinate care for patients in a fast-paced, fully remote environment.
What You'll Do
- Manage a patient census with a resolution-driven approach to close gaps in clinical and non-clinical care.
- Identify and coordinate community resources that would benefit patient care.
- Provide patient education and improve health literacy on the management of chronic conditions.
- Perform medication management, including identifying potential concerns, reconciliation, adherence, and coordinating refills.
- Assist in ensuring timely delivery of services such as Home Health, DME, and Home Infusion.
- Resolve patients' questions and create an open dialogue to understand their needs.
- Assist in managing referrals and appointment scheduling.
What We're Looking For
- Active Compact/Multi-State LPN/LVN license.
- At least 3 years of direct patient-facing work experience.
- Technical aptitude with Microsoft Office Suite.
- Excellent written and verbal communication skills.
- Must have active high-speed Wi-Fi.
- Must have a home office or HIPAA-compliant workspace.
Nice to Have
- Experience with Epic.
- Bilingual skills.
- Additional Single State licensures.
