CareHarmony is looking for a Remote Licensed Practical Nurse (LPN/LVN) - Intake Coordinator to work comprehensively with healthcare providers on value-based care management. In this role, you will conduct preliminary health assessments, coordinate resources, and provide patient education in our fast-paced, remote environment.
What You'll Do
- Accept transfers from the Patient Enrollment team to conduct preliminary health assessments for newly enrolled patients.
- Resolve patients' questions and create an open dialogue to understand their needs.
- Identify and coordinate community resources with patients to benefit their care.
- Provide patient education and health literacy on the management of chronic conditions.
- Assist with medication management, including identifying potential concerns, adherence, and coordinating refills.
- Assist in ensuring timely delivery of services like Home Health, DME, and Home Infusion.
What We're Looking For
- Active Compact/Multi-State license (LPN/LVN).
- At least 3 years of direct patient-facing work experience.
- Technical aptitude – Microsoft Office Suite.
- Excellent written and verbal communication skills.
- Ability to quickly adapt to a fast-paced work environment.
- Willingness to spend most of your shift on the phone.
- Must have active high-speed Wi-Fi.
- Must have a home office or HIPAA-compliant workspace.
Nice to Have
- Epic experience.
- Bilingual skills.
- Additional Single State licensures.

You will accept patient transfers from the Patient Enrollment team as part of our coordinated care workflow.

- Compensation: $21/hr-$28/hr.
