CareHarmony is looking for a Licensed Practical Nurse (LPN/LVN) for a remote Care Coordinator role. You will work comprehensively with providers to deliver value-based care management initiatives, managing a patient census with a resolution-driven approach to close gaps in care, coordinate community resources, and provide patient education.
What You'll Do
- Manage a patient census with a resolution-driven approach to close gaps in clinical and non-clinical patient care.
- Identify and coordinate community resources with patients that would benefit their care.
- Provide patient education and health literacy on the management of chronic conditions.
- Perform medication management, including identifying potential medication concerns, reconciliation, adherence, and coordinating refills.
- Assist in ensuring timely delivery of services to patients, including Home Health, DME, Home Infusion, and other critical needs.
- Resolve patients' questions and create an open dialogue to understand their needs.
- Assist and manage referrals and appointment scheduling.
What We're Looking For
- Active Compact/Multi-State license (LPN/LVN).
- At least 3 years of direct patient-facing work experience.
- Technical aptitude with the Microsoft Office Suite.
- Excellent written and verbal communication skills.
- Must have active high-speed Wi-Fi and a home office or HIPAA-compliant workspace.
Nice to Have
- Experience with Epic.
- Bilingual skills.
- Additional Single State licensures.
