CareHarmony is hiring a Remote Licensed Practical Nurse (LPN/LVN) to join our team as a Care Coordinator. In this role, you will work comprehensively with providers to deliver value-based care management initiatives. You will manage a patient census with a resolution-driven approach to close gaps in clinical and non-clinical patient care.
What You'll Do
- Manage a patient census with a resolution-driven approach to close gaps in clinical and non-clinical patient care.
- Identify and coordinate community resources for patients to benefit their care.
- Provide patient education and health literacy on the management of chronic conditions.
- Perform medication management, including identifying potential concerns, reconciliation, adherence, and coordinating refills.
- Assist in ensuring timely delivery of services like Home Health, DME, and Home Infusion.
- Resolve patients' questions and create an open dialogue to understand their needs.
- Assist and manage referrals and appointment scheduling.
What We're Looking For
- Active Multi-State/Compact License (LPN/LVN)
- At least 3 years of direct patient-facing work experience
- Technical aptitude with the Microsoft Office Suite
- Excellent written and verbal communication skills
- Must have active high-speed Wi-Fi
- Must have a home office or HIPAA-compliant workspace

- Experience with Epic
- Bilingual skills
- Additional single state LPN licensures
