Work remotely as a Licensed Practical Nurse in a pivotal role that bridges patient needs with coordinated care. In this position, you’ll perform initial health evaluations for new patients, gather essential health information, and help shape the direction of their care plans. Your expertise will ensure individuals receive timely access to services such as home health, durable medical equipment, and home infusion support.
Key Responsibilities
- Conduct comprehensive intake assessments over the phone for newly enrolled patients
- Address patient inquiries with empathy and clarity, building trust through active listening
- Educate patients on managing chronic conditions and improving health literacy
- Identify potential medication issues, support adherence, and coordinate refills
- Connect patients with community-based resources to enhance care outcomes
- Ensure seamless transitions and prompt delivery of critical health services
Qualifications
Candidates must hold an active Compact or Multi-State LPN license (NLC) and bring at least three years of direct patient care experience. You should be comfortable working independently in a home-based, HIPAA-compliant environment with reliable high-speed internet.
- Proficiency with Microsoft Office Suite is required
- Strong verbal and written communication skills essential
- Ability to take accurate notes in real time during extended phone conversations
- Must be able to sit for long durations and speak clearly over the phone
Experience with Epic, bilingual abilities, or additional state licenses are valued assets. This role operates full-time Monday through Friday with two shift options: 8:00 AM – 4:30 PM or 9:00 AM – 5:30 PM Central Time. Occasional rotational on-call duty occurs approximately once per year.
